An Enduring Guardianship is a legally binding document that appoints a Guardian to make medical decisions for the person appointing them.
An Enduring Guardianship can appoint more than one guardian. Similarly, an Enduring Guardianship can be revoked at any time.
All parties to the Enduring Guardianship must be over the age of 18, must meet capacity requirements and must not be involved in directly providing your medical care or treatment.
An Enduring Guardianship can provide your instructions for:
- Where you will live
- The level of care you wish to receive
- Approved medical and dental treatments
- End-of-life care
- Any other matter you decide about your general health care
